  •  Make sure your request is complete. Incomplete requests will not be processed.
  • Fill out this form, then print and sign it. You may mail or bring it to the address at the top of this screen.
  •  If you need transcripts sent to more than one recipient, you must fill out a form for each one.
  •  Items marked with an asterisk(*) are not required. All other fields must be completed or your request will not be processed.
  •  Include the appropriate fees with your request. You may pay by Cash (in person), Check, or Money Order. Please make checks payable to the UC Regents..
  •  You may wish to print a second copy of this request for your own records.
  •  As of July 1, 2003, we will no longer accept credit cards as a form of payment for mailed or in-person requests. Please see our web site for alternatives at http://registrar.berkeley.edu.
  •  The Office of the Registrar takes responsibility for handling transcript requests in a prompt and efficient manner. However, we are not responsible for mishandling by the U.S. Post Office or the receiving school or agency. Replacement copies cannot be issued without the normal transcript fee.
  •  If you are unsure about your exact dates of attendance, you may approximate.
  •  If you would like faster service, you may mark "UPS/DHL Service". UPS is next weekday service within the United States. There is an additional $17 for this service. DHL service is for international destinations and is an additional $30. You must request Special Processing for transcripts being mailed DHL or UPS. Special Processing transcripts are $20 per transcript.
  •  An email confirmation will be sent within 24 hours of receipt of your request for Special Processing or Rush requests; 72 hours after receipt of Standard requests. Although an email address is not required in the field above (**), it is highly recommended and is necessary if you wish to receive confirmation.
